Quick Start: Patient Demographic Entry
This Quick Start takes you through
adding a new patient and basic demographics. For more details on how to
select or enter special information please refer to the complete
chapter!
Before proceeding the following databases need to have data:
- PhoneBook-Facility
- PhoneBook-providers
- Insurances
- Procedures
Starting
a New Patient
- Click New
Patient Record from either the Main Menu of Patients or
from
any of its screens and confirm your action at the dialog
- Enter the new patient’s complete
First, MI and Last and click Continue!
- If
there is a possible duplicate patient you get the Duplicate Warning Screen. If
none of these patient’s match the one you are entering click Continue
located on the bottom right of the screen .
- If there are no possible
matches a new patient record will be created, a unique Patient
ID calculated and a Chart number assigned!
- The fields in the Patient Name
section should be completed without any special characters or “carriage
returns”.
- In the Demographics
section
enter the DOB using “/”s (i.e. 8/22/55) and complete any many
fields
as possible. The Age will be calculated
for
you.
- In Status make your
selection
from the pop-up list if the patient is not "Active".
- When he enter the SocSecNo
there is no need to add the dashes as they will be added for you.
- In the Communications
section
enter as many phone numbers as needed. You can leave off the area code
and
the dashes as the software will add them for you.
- If this patient was
referred by a non-patient source, you
as enter it in the Refer by... field. There is an associated
pop-up
list and Edit feature.
- If a current patient
referred this patient, you can enter
that person's Patient ID for later reference.
- In Facility, Providers there are several important fields.
- Facility is the place this
patient is
usually seen at
- The Office Doctor is for doctor under whom all
billings
will be done.
- The Examiner
is the person or entity this patient is served by.
- Primary
Doc is for this patient’s family doctor.
- The Referring Doc is
the physician
who specifically sent in the patient for consultation.
- The Other Doc
can be any other provider involved with
this patient.
- Employer
or Student has
fields
that should be completed if required for claim processing.
- In Diagnoses you
can select or enter unlimited diagnosis
records. You can set the diagnosis priority in the # field,
with "99"
being listed, non used or old diagnosis.
- In Notes the yellow section can stores 64,000
characters,
while the pink section is for a pop-up reminder.
